Contractor paid 2500 GBP to HMRC for tax return mess up

Our question to UK Contractors was:
Has anyone used Accessa Umbrella OR SJD Accountancy services? Can you please share your experiences with us
and we received 3 comments on it.
Everything was fine until Dujon Walsham's comment, an Infrastructure Architect working with UBS Investment Bank wrote, "ive used SJD I recommend them they are very good".
What came after Dujon's comment raised our eyebrows.
Dave Garner MBCS who happens to be a Project Manager wrote in his reply to Dujon's comment that, "Used them for a while and two years later got a letter from HMRC as they allegedly messed up my TAX return and I ended up paying £2500 plus a fine for submitting an incorrect tax return! Found I was doing stuff they should be doing so went to a local accountant about same costs and got a better service and 1:1 sessions which I didn't get with SJD as they are remote."
